LCORE-4742: Implement prompt store operations - #3119
Implement CRUD operations in the prompts module: create (mint pmpt_ id, version 1 as default), get by id with optional version (falls back to default), list all prompts, update (appends immutable version, optionally sets as default), and delete (prompt and all versions). Domain errors surface PromptNotFoundError and PromptValidationError for endpoint handlers to map to HTTP responses. Unit tests cover happy paths, not-found cases, and validation errors.

Walkthrough
This change adds database models for prompts and their versions, plus storage operations to create, retrieve, list, update, and delete prompt records. It also adds prompt exceptions and unit tests for the models and storage operations.
Adds Prompt and PromptVersion models. Tests cover their schema, keys, relationships, and persistence.
Prompt creation and retrieval src/prompts/exceptions.py, src/prompts/__init__.py, src/prompts/store.py, tests/unit/prompts/*
Adds variable-name validation and operations to create, retrieve, and list prompts. Tests cover default and explicit versions, missing records, and duplicate variables.
Prompt updates and deletion src/prompts/store.py, tests/unit/prompts/test_store.py
Updates create sequential versions and can set the default version. Deletion removes a prompt and its versions. Tests cover success and error cases.
The new prompt model test will fail on duplicate-variable validation. Resolve the mismatch before merging.

The title "LCORE-4742: Implement prompt store operations" matches the main change. The PR adds create, get, list, update, and delete operations to the prompt store in src/prompts/store.py, backed by the new Prompt and PromptVersion models and the prompt exception hierarchy. The title names the primary change and includes the ticket ID. It is specific and concise.

Docstring coverage is 100.00% which is sufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff with cyclomatic complexity of at least 2. Documentation-only edits and deletions within surviving functions are included; unchanged diff context and fully deleted functions are excluded. Analyzed 10 functions across 8 files.
Full details: Security And Secret Handling
Explanation
PASSED. The pull request adds models and an internal prompt store; it adds no API endpoint or Kubernetes Secret resource. Store queries use SQLAlchemy parameterized ORM filters, with no raw SQL, shell execution, or path construction. Debug logs contain only prompt IDs and version numbers, not template text, variables, tokens, or credentials. No hardcoded secrets appear in the changed source.
Full details: Performance And Algorithmic Complexity
Explanation
The prior performance finding remains unresolved. src/prompts/store.py:107-116 defines list_prompts() with no pagination or bound and executes session.query(Prompt).all(). This materializes every prompt row, so memory and response cost grow with the catalog. The new list operation has no caller-enforced limit. The repository's existing saved-prompt list uses an explicit upper bound at src/utils/saved_prompts.py:182-202, but the new catalog list does not.
Resolution
Add validated pagination parameters to list_prompts() with a safe maximum limit, offset or continuation support, and deterministic ordering. If pagination is not possible, enforce a documented maximum prompt count and apply that limit in the query.

Add the missing variables validator to PromptVersion. The new model test fails without it.
tests/unit/models/database/test_prompts.py (Lines 283-291, test_duplicate_variables_raises_validation_error) expects PromptVersion(..., variables=["a", "a"]) to raise PromptValidationError with "duplicate names". This model defines no @validates, constructor override, or event hook. SQLAlchemy's declarative constructor only sets attributes, so no exception is raised. pytest.raises fails, and the unit-test run fails.
Add the validator to the model. @validates also runs for constructor keyword arguments.
🐛 Proposed fix
-from sqlalchemy.orm import Mapped, mapped_column+from sqlalchemy.orm import Mapped, mapped_column, validates
from models.database.base import Base
+from prompts.exceptions import PromptValidationError
variables: Mapped[list[str]] = mapped_column(JSON, default=list)
++ @validates("variables")+ def _validate_variables(self, _key: str, value: list[str]) -> list[str]:+ """Reject duplicate variable names.++ Parameters:+ _key: Name of the validated attribute.+ value: Declared variable names.++ Returns:+ list[str]: The unchanged variable names.++ Raises:+ PromptValidationError: If the list contains duplicate names.++ """+ if len(value) != len(set(value)):+ raise PromptValidationError("The variables contain duplicate names")+ return value
If you want validation only in the store layer, remove TestPromptVersionVariableValidation.test_duplicate_variables_raises_validation_error instead. If you add the model validator, _validate_variables in src/prompts/store.py becomes redundant.
As per coding guidelines: "Create unit tests for new code" and "Required for all modules, classes, and functions" (docstrings).

Review comment at @src/models/database/prompts.py at line 45:
Add a `variables` validator to `PromptVersion` so constructor-supplied lists
with duplicate names raise `PromptValidationError` containing “duplicate names”;
return valid lists unchanged. Use the model’s existing validation conventions
and required imports.

Handle concurrent update_prompt calls that compute the same version number.
The code reads the highest version and inserts max_version + 1 in the same transaction without a lock. Two concurrent updates of the same prompt can both compute the same number. The second session.commit() then raises a raw sqlalchemy.exc.IntegrityError on the (prompt_id, version) key. The docstring does not list this error, so endpoint handlers cannot map it to a clean response.
Catch IntegrityError around the commit, roll back, and either retry or raise a domain error. Alternatively, lock the Prompt row with with_for_update() before reading the maximum version.
The Prompt row is also checked only when default=True. For consistency with the docstring, load the Prompt first in every call.

Review comment at @src/prompts/store.py around lines 155 - 159:
Update update_prompt to load and validate the Prompt on every call, not only
when default=True. Prevent concurrent calls from assigning the same version by
locking the Prompt row before reading the maximum version, or handle commit
IntegrityError with rollback and a retry or documented domain error so callers
do not receive a raw SQLAlchemy exception.

Update the prompt timestamp when appending a non-default version.
When default=False, this branch leaves the Prompt row unchanged. Prompt.updated_at therefore does not advance, and list_prompts() returns a stale timestamp after a version is added. Update the parent timestamp in the same transaction for every appended version. SQLAlchemy applies onupdate only when it updates that row. (docs.sqlalchemy.org)

Validate the prompt name before inserting it.
If name exceeds 255 non-space characters, PostgreSQL rejects the insert into Prompt.name at session.commit(). create_prompt then exposes a database error instead of PromptValidationError. Reject the name before creating either row, and cover the limit in a store test. (postgresql.org)

Ensure docstrings follow the project’s established "Parameters:" header format for any documented function parameters.
Introduce Prompt and PromptVersion ORM models for the local prompt
catalog. Prompt holds identity and a default-version pointer;
PromptVersion uses a (prompt_id, version) composite primary key for
immutable version rows with template text and declared variables.
Includes the prompts module skeleton with domain exceptions and unit
tests covering schema definition, constraints, and variable validation.
This makes sure both tables are registered when we import Base at
src/app/database.py. All the other tables (i.e. SavedPrompt,
UserConversation, and UserTurn) are registered transitively, so
it will be better we also import those tables at __init__.py.
Implement CRUD operations in the prompts module: create (mint pmpt_ id,
version 1 as default), get by id with optional version (falls back to
default), list all prompts, update (appends immutable version, optionally
sets as default), and delete (prompt and all versions). Domain errors
surface PromptNotFoundError and PromptValidationError for endpoint
handlers to map to HTTP responses. Unit tests cover happy paths,
not-found cases, and validation errors.
