graph: add totalCount to entity followers

• Add totalCount to app.certified.graph.listEntityFollowers, counting distinct actor–entity relationships before cursor and limit, including on empty pages.
• Keep listEntityFollowing behavior unchanged.
• Refresh the Lexicon, explorer and bundled consumer docs; add a minor Changeset.

Validation

• pnpm check — passed (347 unit tests, 17 explorer tests).
• pnpm build — passed.
• pnpm changeset status and git diff --check — passed.
• HTTP runtime tests could not start: preflight found the pinned PostgreSQL image missing. No image pull or Compose startup occurred.
