Add a table to track BSA-ready Tlds - #3255

To support new BSA enrollment, we must distinguish between those that have been initialized and ready and those that are yet to be initialized. We are adding a new table to record the TLDs that are ready, and let the `Tld` table track all enrolled TLDs.
