Preserve midstream BOMs when flushing before strings
When a binary chunk has already been decoded, a nonempty string is written, and the next binary chunk begins with EF BB BF, this zero-argument decode() resets the TextDecoder; the next decode therefore treats those bytes as a new leading BOM and suppresses them. Previously the decoder stayed in streaming mode and emitted the midstream U+FEFF, so this sequence now silently drops parser input; flush the incomplete bytes without losing the decoder's already-started/BOM state.

- Around line 34-36: Update the write logic around `hasBinaryInput` to track
whether a nonempty string chunk has already been written; when binary input
first follows that text, use a `TextDecoder` configured with `ignoreBOM: true`
so its leading BOM is preserved. Keep the existing behavior for binary input
that precedes any text.

Both writable stream adapters accept decoded string chunks alongside binary chunks. If a UTF-8 character is incomplete when a nonempty string arrives, the decoder retains the bytes until a later binary chunk. For
[0xe2,0x82],"text",[0xac], callbacks therefore emittext€, moving earlier bytes after the string.Flush pending decoder bytes before nonempty string chunks in the Node and Web adapters. Empty strings retain the decoder state so they do not split a valid fragmented character. Tests pin ordering and the empty-string case for both APIs.
Validation: both ordering regressions fail before the change.
