fix(openai-native): send body prompt_cache_key for cache affinity - #2253
The OpenAI-native handler now sends a bodyprompt_cache_key on both Responses request builders (streaming createMessage and non-streaming completePrompt), and no longer sends the session_id request header.
Before
After
Cache affinity
session_idheader (ignored for caching)
prompt_cache_keybody field
Streaming body
no key
prompt_cache_key: <taskId>
completePrompt body
no key
prompt_cache_key: <handler sessionId>
prompt_cache_retention: "24h"
sent on the 7 pre-5.6 catalog models
unchanged
Why
OpenAI's prompt-caching guide documents prompt_cache_key as the body field that controls cache routing: "On models before GPT-5.6, use a stable prompt_cache_key for requests that share a reusable prefix to help route related requests to the same cache." On GPT-5.6 and later the key is optional but still useful for separate cache accounting per user/workspace.
The session_id header we were sending is not part of the prompt-caching API. OpenAI's own codex tracker records it as "ignored/dropped" on the public Responses API. Two consequences:
Every OpenAI-native task left cache hit-rate on the table, because routing had no key to group a task's turns under.
The header name contains an underscore. Hardened gateways (Envoy/nginx underscores_in_headers: false) reject such headers outright, so it was a latent 400 for anyone pointing openAiNativeBaseUrl at a strict proxy.
Sources:
OpenAI prompt caching guide — prompt_cache_key semantics, the before/after-GPT-5.6 split, and the separation-of-accounting use.
OpenAI codex#14312 — "we're sending the header session_id to responses which is ignored/dropped".
Stability contract
The key is metadata.taskId, which is the persisted task id (Task.taskId, a uuidv7 generated once per task and stored with its history). That makes it:
Stable across turns of one task — the growing prefix keeps landing on the machine that holds its cached KV state.
Stable across restarts and rehydration — the id is read back from history, not regenerated.
Distinct across tasks — unrelated prefixes do not compete for one routing bucket.
The key is task-scoped only. It is sent when, and only when, metadata.taskId exists. Requests with no task id send noprompt_cache_key at all, so OpenAI's default routing applies unchanged.
That deliberately includes the completePrompt path. It carries no task id (CompletePromptOptions is only abortSignal/timeoutMs) and its callers (singleCompletionHandler) build a throwaway handler per call, so no conversation exists whose prefix could accumulate. An earlier revision of this PR filled the field with the handler's per-instance uuidv7(); review flagged that as a per-call random key, which buys zero affinity while replacing whatever routing OpenAI applies by default. Whether that default is prefix-hash routing is not stated in the prompt-caching guide, so the premise is unverified, and the fail-safe for an unverified premise is to leave default routing untouched. The field may be revisited once maintainers have routing data for the single-completion path.
The key is omitted entirely for models whose catalog entry has supportsPromptCache: false, and clamped to OpenAI's 64-character limit (task ids are 36 chars today, so this is a guard, not routine truncation).
prompt_cache_retention is untouched
prompt_cache_retention: "24h" is a supported lifetime control on the seven pre-5.6 models that set promptCacheRetention: "24h" in the catalog (gpt-5.1*, gpt-5.2*, gpt-5.3-codex). OpenAI's compatibility table is generation-scoped: prompt_cache_options.ttl is the 5.6+ control, prompt_cache_retention is the control for everything earlier. The existing retention specs pass unmodified, and a new test asserts the key and the retention field coexist on the same request.
Scope
Only src/api/providers/openai-native.ts and its two spec files. Adjacent-pattern sweep for the header literal across the repo found:
openai-codex.ts — keeps its session_id header (the ChatGPT/Codex backend does honor it) and already sends a body prompt_cache_key on its Responses Lite path. Untouched.
xai.ts, opencode-go.ts — never had the header. Untouched.
No usage-accounting changes (that is PR-C1/C9). No changeset, no CHANGELOG.
Disclosure
The completePrompt path sends noprompt_cache_key, so single-completion requests get no cache affinity from this PR. This is the fail-safe choice pending maintainer routing data: we do not overwrite OpenAI's default routing with a key that cannot help. The handler's sessionId field and its uuidv7 import were removed as dead code once the fallback went.
Testing
Targeted: openai-native.spec.ts + openai-native-usage.spec.ts → 121 passed. All ten affected provider suites → 477 passed.
New tests fail on base: with the production file stashed, 9 of the new tests fail.
Full suite: Test Files 1 failed | 551 passed | 3 skipped (555) / Tests 1 failed | 11945 passed | 37 skipped (11983). The single failure is mistral.spec.ts > completePrompt > keeps TimeoutError when the timeout fires before a later caller abort, which reproduces on a pristine origin/main tree with this branch's changes stashed (verified twice) — it is a pre-existing bug from origin/main tip 159c42e15 (feat(api): abort signal support for gemini, mistral, lite-llm (completePrompt + createMessage) #1303), not from this PR.
tsc --noEmit clean. ESLint clean on all three files with no suppression entries added (the new tests use bracket notation instead of as any).

Improvements
Streaming requests now use a consistent prompt-cache key for the same task and distinct keys for different tasks.
Cache keys are limited to 64 characters and omitted when a task ID is unavailable or the model does not support prompt caching. Non-streaming requests do not include a cache key.
The 24-hour cache retention setting is preserved, and requests no longer send a session ID header.
OpenAI Native streaming requests now include a task-scoped prompt_cache_key for supported models. The handler truncates keys longer than 64 characters and omits the key when no task ID or model support exists. SDK and fetch requests no longer send a session_id header.
Changes: Prompt-cache key routing
Layer / File(s)
Summary
Select and send prompt-cache keys src/api/providers/openai-native.ts, src/api/providers/__tests__/openai-native*.spec.ts
Supported streaming requests use the task ID as prompt_cache_key, truncated to 64 characters when needed. Requests omit the key when no task ID or model support exists. Tests cover key stability, key omission, request paths, and 24h retention.
Remove session ID headers src/api/providers/openai-native.ts, src/api/providers/__tests__/openai-native.spec.ts
The handler no longer generates a session ID or sends session_id headers through SDK and fetch requests. Tests check that the headers are absent.
No merge-blocking issue is established for the task-scoped cache keys or session-header removal. Mergeable subject to normal checks.

Full details: Regression Evidence
Explanation
The changed request behavior has focused coverage. openai-native-usage.spec.ts exercises task-key creation, same-task stability, distinct-task separation, missing-task omission, unsupported-model omission through createMessage, 64-character truncation, and retention coexistence. openai-native.spec.ts also covers both SDK and manual SSE paths, varying conversation input, omission without metadata, and removal of session_id from per-request and SDK default headers. completePrompt has focused omission tests for supported and unsupported models. No changed behavior lacks a plausible regression test, and no UI snapshot is applicable.
Full details: Persistence Integrity
Explanation
The changed production path only derives an optional prompt_cache_key from metadata.taskId and changes request headers. The diff contains no task/history persistence reads or writes, asynchronous persistence operation, atomicity-sensitive write, rollback path, or persisted default propagation. Therefore no changed path meets the stated persistence-integrity failure condition.
Full details: Lifecycle Resource Cleanup
Explanation
PASS — The changed production path only removes session_id state/header handling and adds a pure prompt_cache_key body field with task-ID gating. The diff adds no listener, watcher, timer, task, or new resource allocation, and it does not alter cancellation, stream cleanup, or restart behavior. The existing AbortController and finally cleanup paths remain unchanged.
Full details: Security Boundaries
Explanation
The security finding remains unresolved. The changed provider path sends metadata.taskId to OpenAI as prompt_cache_key after only a truthiness check and truncation (src/api/providers/openai-native.ts:394-395,1449-1458). The value is not validated as a task ID. webviewMessageHandler.ts:703-714 forwards message.taskId; Task.ts:723 accepts a caller-supplied ID unchanged; and apps/cli/src/agent/extension-host.ts:517-529 exposes an unchecked taskId parameter. For example, a caller can pass alice@example.com or reuse another task ID. The provider then sends the value to OpenAI or assigns unrelated tasks to the same cache-affinity bucket. The added tests use arbitrary values such as task-abc and do not reject malformed IDs.
Resolution
Validate task IDs at every applicable webview and API ingress, and validate again before constructing prompt_cache_key. Accept only the explicit format used by generated or persisted task IDs, such as the repository's UUID format. Omit the field for invalid or absent IDs. Keep the 64-character limit as a separate provider constraint. Add tests for malformed IDs, overlong IDs, and cross-task task-ID reuse.
Full details: Description check
Explanation
The description provides detailed rationale, implementation scope, and test results. However, it omits the required approved GitHub Issue link and pre-submission checklist. It also contains conflicting statements about whether completePrompt sends prompt_cache_key.
Resolution
Add a valid Closes: #<issue-number> entry, complete the required pre-submission checklist, and correct the conflicting completePrompt statements so the description consistently states that this path sends no prompt_cache_key.

What: the OpenAI-native handler now sends a task-scoped `prompt_cache_key`
in the Responses request body on both body builders (streaming and
completePrompt), and no longer sends the `session_id` request header.
The key is `metadata.taskId`, falling back to the handler session id,
clamped to OpenAI's 64-character limit, and omitted for models whose
catalog entry has `supportsPromptCache: false`.
Why: OpenAI keys prompt-cache routing off the body `prompt_cache_key`
field. The `session_id` header we were sending is not part of the
prompt-caching API and is ignored on the public Responses API, so every
OpenAI-native task was leaving cache hit-rate on the table. The header
also carries an underscore in its name, which hardened gateways
(Envoy/nginx `underscores_in_headers`) reject outright, so it was a
latent 400 for anyone behind such a proxy with a custom base URL.
`prompt_cache_retention: "24h"` is untouched: it is the supported
lifetime control on the pre-5.6 models that set it in the catalog, and
the existing retention specs pass unmodified.
Impact: cache affinity now follows the documented body field, stable
across turns of one task and distinct across tasks. No usage
accounting, cost, or other request field changes. openai-codex.ts and
xai.ts are untouched: codex already sends a body key, and xai never had
the header.

What: the prompt cache key is now sent ONLY when `metadata.taskId` exists.
The `completePrompt` path omits the field entirely instead of filling it
with the handler's per-instance `uuidv7()`. The now-unused `sessionId`
field and its `uuidv7` import are removed.
Why: review of Zoo-Code-Org#2253 flagged that the session fallback put a per-call
random key on the wire. `completePrompt` carries no task id
(`CompletePromptOptions` is only abortSignal/timeoutMs) and its callers
(`singleCompletionHandler`) build a throwaway handler per call, so no
conversation exists whose prefix could accumulate. A random key therefore
buys zero affinity while replacing whatever routing OpenAI applies by
default. Whether that default is prefix-hash routing is not something the
prompt-caching guide states, so the premise is unverified; for an
unverified premise the fail-safe is to leave default routing untouched
rather than overwrite it with a value we know cannot help.
Behavior note: `prompt_cache_key` is task-scoped only. Requests without a
task id send no key, so OpenAI's default routing applies unchanged. The
field may be revisited once maintainers have routing data for the
single-completion path.
Impact: no change to the `createMessage` paths, which always carry a task
id, so task-level affinity is unaffected. `prompt_cache_retention: "24h"`
is still untouched. No usage-accounting, cost, or other request-field
changes.
The no-task test calls buildRequestBody("gpt-5.1") directly and does not exercise the streaming entry point. The streaming path reaches the same builder through createMessage, which forwards metadata to handleResponsesApiMessage, and that method passes metadata into buildRequestBody. If a fallback task ID is added on that path, this test can still pass. Add a test that calls handler.createMessage(systemPrompt, messages) without metadata, then asserts that the request body has no prompt_cache_key.

What: add a createMessage-level test asserting that the Responses request
body carries no prompt_cache_key when no metadata is supplied.
Why: the omission was only pinned through the private buildRequestBody and
through completePrompt, which builds its own body inline and never reaches
buildRequestBody. A fallback task id introduced on the streaming path
(createMessage -> handleResponsesApiMessage -> buildRequestBody) would have
passed both. The new test fails on exactly that mutant.
Impact: test-only, +13 lines, no production change. Stryker's changed-line
budget excludes __tests__, so the 12/500 gate figure is unchanged.

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.
Preserve cross-task cache reuse on GPT-5.6 and later.
Different task IDs create separate cache-accounting groups. When two tasks share an eligible cached prefix, the second task loses the prior cache-read credit. OpenAI states that these models do not need a key for cache routing. (developers.openai.com)
Omit the key for these models, or use a stable user or workspace key when separate accounting is required.

What:
- openai-native-usage.spec.ts: add a createMessage test with codex-mini-latest
and a task id, asserting the SDK request body omits prompt_cache_key. The
spec now stubs the openai client so the public entry point is drivable.
- openai-native.spec.ts: extend the SDK-path header test to also assert the
OpenAI constructor's defaultHeaders carry neither session_id nor
prompt_cache_key.
Why:
Both existing assertions stopped one level short of the surface that ships.
The builder-level model-gate test cannot see createMessage, which forwards
metadata into buildRequestBody, so a key injected anywhere along that path
still passed. The per-request header assertion cannot see defaultHeaders, so a
restored affinity header there would ship on every request while the test
stayed green. Each new assertion was proven to fail against a mutant that
plants exactly the value it forbids (streaming-path key injection; session_id
and prompt_cache_key in defaultHeaders), and each mutant was killed by the new
assertion alone.
Impact:
Test-only. Zero production lines; prompt_cache_key semantics unchanged. No new
ESLint suppressions, no changeset.
Rebuttal — openai-native.ts:1458, "Preserve cross-task cache reuse on GPT-5.6 and later"
Verdict: not changing this. Per-task keying is the deliberate design, and the suggested alternative is not safe on this catalog. CodeRabbit marked this one Unverified finding and Minor, and verification is right to have failed: the finding's premise is contradicted by the vendor doc it cites.
1. The recommendation would break 37 of 44 catalog models
The finding proposes omitting the key, or using "a stable user or workspace key", for GPT-5.6 and later. The code has no generation gate, and it should not get one:
"OpenAI handles cache routing automatically; the key is not needed to optimize caching" — but "you can use separate keys to maintain separate cache accounting"
Omitting the key to help the 7 would forfeit the documented routing lever on the 37. A "stable user or workspace key" is worse than omission on both slices: on pre-5.6 it sends a single key for all of a user's traffic, which the same doc warns about ("For busy groups, aim for about 15 requests per minute in total across all prefixes using each key. Partition higher-volume traffic across multiple keys"), and on 5.6+ it merges accounting across every task that user runs.
2. Per-task keying is the design, not an accident
The key is metadata.taskId, the persisted task id, chosen because it is the only identifier stable across turns, restarts, and rehydration and distinct per task. That decision is recorded in the PR body ("Stability contract"), in coder-report-pr-c2.md §2/§7, and in the C2 ledger row of docs/261031_0001_session_caching-diet-wave/fix-list-caching-1.md. The prior review round already rejected a per-call random key for the same reason in reverse: an identity that does not correspond to a conversation buys no affinity while replacing default routing.
3. Task-scoped accounting is a requirement of the C5a family, not a side effect
cacheWriteTokens is accumulated per task and attributed per task on two surfaces:
src/core/task/Task.ts:4072 — cacheWrites: cacheWriteTokens written into the task's own api_req_started message, i.e. the number the user sees and the number that persists in task history.
src/core/task/Task.ts:4533-4539 — TelemetryService.instance.captureLlmCompletion(this.taskId, {... cacheWriteTokens: tokens.cacheWrite, ...}), keyed by this.taskId.
The C5a work (coder-report-pr-c5a.md, ADR-0006 phase 1) splits cache-write billing into 30m/12h classes so a write can never be billed at the wrong TTL rate. That split is only meaningful if writes are attributable to the task that caused them. A workspace-wide key would let one task's prefix be credited to another's accounting, which is exactly the merge the finding calls "cross-task cache reuse" and exactly what the accounting family exists to prevent. The finding's own framing concedes the mechanism: "Different task IDs create separate cache-accounting groups."
4. The claimed loss is unquantified, and the vendor offers a different lever
"Two tasks sharing an eligible cached prefix" is rare in this product by construction: a task's prefix is its own system prompt plus its own growing conversation, and per the same doc a timestamp or any user-specific byte before the breakpoint makes the prefix ineligible anyway. Meanwhile the vendor's actual GPT-5.6+ efficiency levers are prompt_cache_options.mode / prompt_cache_breakpoint / ttl, which this PR deliberately does not touch (recorded as rows K5-K7 in coder-report-pr-c2.md §"Not in this PR"). If cross-task reuse turns out to matter, the correct fix is breakpoint placement, not a key that erases per-task accounting.
5. Guards are already pinned
The key is omitted for supportsPromptCache: false models and when no task id exists, and clamped to 64 chars. Those three gates are asserted at builder level and, as of 8d1d1e44d, on the streaming entry point and the SDK constructor's defaultHeaders too.
If a maintainer wants per-workspace accounting groups on 5.6+ models specifically, that is a product decision with a real cost (per-task attribution on those 7 models) and it should arrive as its own unit with the accounting change, not as a cache-affinity tweak inside this PR. Until then this PR keeps the documented, task-scoped behavior.

prompt_cache_keyon both Responses request builders (streamingcreateMessageand non-streamingcompletePrompt), and no longer sends thesession_idrequest header.session_idheader (ignored for caching)prompt_cache_keybody fieldprompt_cache_key: <taskId>completePromptbodyprompt_cache_key: <handler sessionId>prompt_cache_retention: "24h"Why
OpenAI's prompt-caching guide documents
prompt_cache_keyas the body field that controls cache routing: "On models before GPT-5.6, use a stableprompt_cache_keyfor requests that share a reusable prefix to help route related requests to the same cache." On GPT-5.6 and later the key is optional but still useful for separate cache accounting per user/workspace.The
session_idheader we were sending is not part of the prompt-caching API. OpenAI's own codex tracker records it as "ignored/dropped" on the public Responses API. Two consequences:underscores_in_headers: false) reject such headers outright, so it was a latent 400 for anyone pointingopenAiNativeBaseUrlat a strict proxy.Sources:
prompt_cache_keysemantics, the before/after-GPT-5.6 split, and the separation-of-accounting use.Stability contract
The key is
metadata.taskId, which is the persisted task id (Task.taskId, a uuidv7 generated once per task and stored with its history). That makes it:The key is task-scoped only. It is sent when, and only when,
metadata.taskIdexists. Requests with no task id send noprompt_cache_keyat all, so OpenAI's default routing applies unchanged. That deliberately includes the
completePromptpath. It carries no task id (CompletePromptOptionsis onlyabortSignal/timeoutMs) and its callers (singleCompletionHandler) build a throwaway handler per call, so no conversation exists whose prefix could accumulate. An earlier revision of this PR filled the field with the handler's per-instanceuuidv7(); review flagged that as a per-call random key, which buys zero affinity while replacing whatever routing OpenAI applies by default. Whether that default is prefix-hash routing is not stated in the prompt-caching guide, so the premise is unverified, and the fail-safe for an unverified premise is to leave default routing untouched. The field may be revisited once maintainers have routing data for the single-completion path. The key is omitted entirely for models whose catalog entry has
supportsPromptCache: false, and clamped to OpenAI's 64-character limit (task ids are 36 chars today, so this is a guard, not routine truncation).prompt_cache_retentionis untouchedprompt_cache_retention: "24h"is a supported lifetime control on the seven pre-5.6 models that setpromptCacheRetention: "24h"in the catalog (gpt-5.1*,gpt-5.2*,gpt-5.3-codex). OpenAI's compatibility table is generation-scoped:prompt_cache_options.ttlis the 5.6+ control,prompt_cache_retentionis the control for everything earlier. The existing retention specs pass unmodified, and a new test asserts the key and the retention field coexist on the same request.Scope

src/api/providers/openai-native.tsand its two spec files. Adjacent-pattern sweep for the header literal across the repo found:openai-codex.ts— keeps itssession_idheader (the ChatGPT/Codex backend does honor it) and already sends a bodyprompt_cache_keyon its Responses Lite path. Untouched.xai.ts,opencode-go.ts— never had the header. Untouched.packages/cloudcreated_session_id— unrelated OAuth field. No usage-accounting changes (that is PR-C1/C9). No changeset, no CHANGELOG.
Disclosure
completePromptpath sends noprompt_cache_key, so single-completion requests get no cache affinity from this PR. This is the fail-safe choice pending maintainer routing data: we do not overwrite OpenAI's default routing with a key that cannot help. The handler'ssessionIdfield and itsuuidv7import were removed as dead code once the fallback went.Testing
