Completes the last four Open Source Accessibility Framework actions on tracking issue 55.

Supported environments and Known limitations now say what has and has not been run. The framework count is 24 of 24.

Checks: npm run build:html (201 of 201 files) and npm run check:accessibility (no new findings).

Generated with Claude Code

Four Open Source Accessibility Framework actions are now met.

Dependencies and upstream blockers: both known blockers are reported
upstream, issue 70 as GitHub Community discussion 211602 and issue 175
as microsoft/vscode issue 345026, and both are linked in ACCESSIBILITY.md.

Screen reader spot check: the accessibility owner's own run of the
website's core tasks, on his test machine (Windows 11 Pro, JAWS 2026
and NVDA 2026.2, Chrome 156 and Edge 156), is recorded as the result.

Manual checks: an agent's manual visual run of tasks 1 to 4 on the live
site, accepted by the accessibility owner. It found two barriers, the
breadcrumb cut off at 320 CSS pixels (280) and unexplained
abbreviations in Chapter 05 (279).

Recognizing contributions: past accessibility contributions, which
predate the asking workflow, are credited by public GitHub handle.

Supported environments and Known limitations now say what has been run
and what has not.
